The consumer asks to be contacted
They open the quote form on this website and enter their name, phone number, email address, date of birth, state, what they are interested in and how much coverage they want. Naming a beneficiary is optional.
They tick a consent box that starts out unchecked
The box sits on the same form, below the fields and just above the Continue button, with its full wording beside it. Directly under it is a short notice saying who may call and text, that consent is not a condition of purchase, that rates may apply, how to reply STOP or HELP, and where to reach support, with links to the privacy policy, the SMS terms, this page and the accessibility statement. The form goes no further until the box is ticked, and ticking it is not bundled into agreeing to anything else. Its wording is reproduced below.
They choose an appointment time, or skip
Next they can pick a time from the calendar, or skip it and ask to be contacted instead.
We text a six-digit code to the number they gave
The code proves the number belongs to the person who entered it. The screen that asks for it shows the full consent disclosure, reproduced below, above the Verify button. Entering the code and clicking Verify confirms that consent and is the consumer’s electronic signature on it.
What was shown is what is stored
When the code is entered, the disclosure shown above the Verify button is saved word for word with the date, time, IP address and browser. Reply STOP at any time to end the messages, or HELP for contact details.
The consent checkbox, exactly as it appears on the form
By checking this box, I agree that Citadel Heritage may call and text me (including by automated means, AI, or prerecorded/artificial voice) at the number provided about insurance. Consent is not a condition of purchase. Message & data rates may apply; message frequency varies. Reply STOP to opt out or HELP for help. I confirm I am at least 18 and the subscriber or authorized user of this number. See our Privacy Policy (https://citadelheritage.com/privacy) & SMS Terms (https://citadelheritage.com/terms).
The disclosure, exactly as it appears above the Verify button
By entering the 6-digit code and clicking Verify, I provide my express written consent and electronic signature authorizing Jonathan Barnes, a licensed insurance agent (NPN 21719380) (and those calling or texting on their behalf) to contact me at the phone number I provided — including by automatic telephone dialing system, by artificial, prerecorded, or AI-generated voice, and by autodialed or automated text (SMS) messages — and by email, for marketing and insurance follow-up regarding life insurance and related products. I understand these communications may be delivered using automated technology, and that calls may be recorded, in which case I am told so at the start of the call. I am not required to provide this consent (directly or indirectly) as a condition of purchasing any property, goods, or services. Message frequency varies and message and data rates may apply. Reply STOP to opt out of texts at any time, or HELP for help; for help you may also contact Jonathan Barnes at +16187268581 or jonbarnesffl@gmail.com. I can revoke this consent at any time. I confirm I am the subscriber or customary user of this phone number and that I am 18 or older. See our Privacy Policy (https://citadelheritage.com/privacy) and Terms (https://citadelheritage.com/terms).
Other ways a number reaches the program
A personal booking link, emailed by Citadel Heritage to an address the consumer gave, opens the booking page with their number already on file. A consent box is shown there too, but it is optional: the consumer can book without ticking it. A code is texted to the number on file, and the screen that asks for it shows the same full disclosure quoted above. Entering the code stores that disclosure, with the date, time, IP address and browser, as their consent, whether or not the box was ticked. The box on that page reads: "By checking this box, I agree that Citadel Heritage may call and text me (including by automated means, AI, or prerecorded/artificial voice) at the number provided about insurance. Consent is not a condition of purchase. Message & data rates may apply; message frequency varies. Reply STOP to opt out or HELP for help. I confirm I am at least 18 and the subscriber or authorized user of this number. See our Privacy Policy & SMS Terms."
A consumer who asks to be contacted through a lead form on a Facebook or Instagram ad that names Citadel Heritage is not enrolled in this program by that form. They join only in one of the two ways described above.
A consumer who asks for a text during a call receives only the text they asked for, plus confirmations and reminders for an appointment they book. That does not enrol them in marketing messages.
A number bought from a lead provider is never enrolled in marketing texts because of that purchase.
A consumer who texts in may get replies from the assistant, and one message checking back about a day later if the conversation stops before a time is booked.
Citadel Heritage is a d/b/a of Jonathan D Barnes, a licensed insurance agent, NPN 21719380. This is the page the carrier filing points at, so the name on the filing is named here.
